Brenda supervises all departments and oversees all operations at Chestelm Health & Rehabilitation Center. She is ultimately responsible for the welfare of each and every resident here.
Family members, residents, or responsible persons are encouraged to call if they should have any questions or concerns relating to any part of the functioning of the facility or any of its employees.

Nancy Congdon

Nancy has over 20 years of experience in long term care, dementia/Alzheimer’s care, and post-acute short term care. She coordinates our residents day to day lives at Chestelm to ensure we are meeting their psycho-social needs and customary routines. Nancy is also actively involved in our discharge planning for our short term patients. She also coordinates our Family Council Group. Please call Nancy for any questions or concerns.
Chief Financial
Officer Mark Epright, JD, LLM
Mark can assist you with any questions you may have regarding the financial decisions of the facility. Mark also serves as a Compliance Officer. Any "red flag" issues should be brought to his attention immediately.
Director Nursing Services
Jason Yarbrough
Jason is our Director of Nursing Services. He is responsible for the daily management of all patient care. He oversees the entire Nursing Department. Jason communicates closely with our Medical Team of providers as well as our residents/patient, families, resident representatives and employees.
Rehab Manager
Erica Drake
Erica oversees the daily operations and treatments provided by the Rehab Department, including coordination of occupational, physical, and speech therapy services. Erica works closely with the nursing staff to monitor the status of all residents at Chestelm Health & Rehabilitation Center.
Erica ensures that a rehab assessment is completed quarterly on all residents to ensure their highest level of functioning.
MDS

Kaitlin works closely with our Director of Nurses in the completion of our MDS’s. The MDS is a powerful tool for implementing standardized assessments and for facilitating care management in nursing homes. The MDS is used for many quality initiatives.
Admission Coordinator
Christine Dombroski, LPN
Christine can assist you with the admission process and discharge planning. Christine welcomes you to Chestelm and will assist you with getting settled, and any admitting or discharge concerns.
Therapeutic Recreation Director & Volunteer Coordinator
Marie Sola
Marie supervises an astonishing variety and volume of recreational and social activities at Chestelm Health & Rehabilitation Center.
Additionally, she oversees the following programs: Alzheimer's Support Group (Dementia Consultant), palliative care and holistic care coordinator, reiki master, and music therapy volunteer coordinator.
Food Service Supervisor
Amanda DeTour
Amanda oversees the Dietary Department. She is a certified Food Service Director. She continually meets with all of our residents regarding their food concerns or preferences. You can contact Amanda if you are interested in planning a special lunch or family party.
Rebecca Iselin, RD
Rebecca visits all residents upon admission to assure the resident is on the most appropriate diet. Every effort is made to accommodate food preference and maintain the residents' usual eating pattern. All residents are constantly reviewed to monitor their overall nutritional status.
Facilities Director
Charles Termini
The Environmental Coordinator oversees the Maintenance Department, Housekeeping Department, and Laundry Department of Chestelm Health & Rehabilitation Center. Charles is responsible for fire safety and all safety issues pertaining to our residents, staff, and visitors. Contact him with any concerns.
Brendon Bourcier
Infection Preventionist
Brendon is responsible for infection control and prevention for residents and healthcare workers so we can reduce the risk of healthcare-associated infections. He routinely analyzes, provides surveillance, educates, and communicates all needs for the best possible care and promotes a safety culture.
